what is a family health team?
The North Simcoe Family Health Team is a not-for-profit primary health care organization located in the town of Midland, Ontario. It is extremely difficult for an individual health care professional from any one discipline to provide truly comprehensive, holistic care. An inter-professional approach to treatment enables individuals from various disciplines to share unique perspectives to achieve a common goal – the augmentation of the current health status of a patient or client.
Thus, in order to address all aspects of a patient’s well being, health care workers must integrate information and work as a team. One solution to improving this much needed communication and collaboration between health care professionals is the model of health care delivery adopted by the North Simcoe Family Health Team.

Family Health Teams are health care organizations that include a team of family physicians, nurse practitioners, registered nurses, social workers, dietitians, and other professionals who work together to provide health care for their community. Family Health Teams provide more service and a wide range of health options.
Family Health Teams ensure that people receive the care they need in their communities, as each team is set-up based on local health and community needs. They focus on chronic disease management, disease prevention, and health promotion, and work with other health care organizations, such as public health units and Community Care Access Centres.

can I use your programs?
The services of the North Simcoe Family Health Team are only available to patients rostered to our doctors. However, any individual with diabetes can access the services provided by the Diabetes Education Program.
