Well Woman Program
Who we are
The Women’s Wellness Program was developed by the North Simcoe Family Health Team (NSFHT) in response to an identified need to improve access to quality primary care for specific populations.
The goal for this program is:
To assist women to achieve, sustain and maintain health as defined by the woman herself, to her full potential.
Program Overview
A clinic is offered two times per month with 8 clients booked each clinic day. Each Client is assessed by a Registered Social Worker, Dietitian and Nurse Practitioner using evidence based tools and best practice guidelines for health promotion and illness prevention.
We provide holistic, comprehensive and a multidisciplinary approach to woman’s health. It consists of screening and referring to appropriate services as required. This includes:
- Mental Health assessment (stress identification, depression/anxiety screening, addiction, domestic violence)
- Fracture Risk assessment, nutritional screening
- Routine physical health screening – FOBT, mammogram, Bone Mineral Density, immunization, breast exam, PAP
