Who we are

The Well Teen Clinic was established in 2009 and consists of a multi-disciplinary team that includes: 

• Social Worker
• Nurse Practitioner
• Dietitian

Well Teen is available at the North Simcoe Family Health Team office and once a month in Elmvale.

Program Overview

At the North Simcoe Family Health Team we recognize the unique health needs of adolescents.

What should I expect at an appointment?

Each appointment is booked for 45-60 minute.  It is a confidential appointment and designed to provide a holistic, multidisciplinary approach to health screening and education for young people ages 13-21. 

Three areas discussed/reviewed during your appointment are:

Physical health:  The nurse practitioner will meet with you to discuss your overall health including topics such as birth control, sexual health, and skin care.

Emotional Health: The social worker will explore questions/concerns related to relationships, mood and anxiety, and the use of drugs and alcohol

Nutritional health: The dietitian will explore topics such as body image, nutrient requirements for teenagers, and healthy eating habits.
