Seniors Wellness Clinic
Who we are
The Seniors Wellness Clinic is the newest addition to the NSFHTs Health Promotion Program. The Seniors Team consists of:
- Administrative Assistant
- Social Worker
- Nurse Practitioner
- Employee of the Arthritis Society
The Seniors Wellness Clinic is a screening program for people over the age of 65 who would like a comprehensive, holistic assessment.
Access to the clinic is by referral from your physician or nurse practitioner. At the time of referral, your doctor/nurse will give you a questionnaire to take home and bring, completed with you to your appointment. The NSFHT administrative assistant will call you with a date and time.
What can I expect at my appointment?
Plan on being in the clinic for about 1 hour.
Your appointment will consist of first meeting the social worker. She will conduct a memory and a mood test and review your questionnaire with you.
You will then meet with our pharmacist. Please bring all your medication to the appointment, including over-the-counter and herbal supplements. The pharmacist will ensure that you are taking the correct medication in the right manner.
The next person you will meet is our nurse practitioner. The nurse practitioner will conduct a physical exam. After your exam, the dietitian will spend a few minutes with you assessing your nutritional status.
When available, a physiotherapist will conduct a brief falls assessment. The results of your assessment will be documented in your chart and shared with the doctor/nurse practitioner.
Follow-up appointments will be arranged as required.
