Healthy Living Series
Who we are
The Health Living Series was initiated in 2008. The goal of the program is to identify and improve outcomes for individuals as risk for chronic disease, and provide education that supports individual learning and self management of their health. This program is facilitated by a multidisciplinary team including:
- Registered Nurse Educators
- Social Worker
- Dietitian
- Pharmacist
Program Overview
Through a series of four classes information provides focuses on risk factors for:
- Diabetes and heart disease
- Healthy Eating and Lifestyle
- Heart Health
- Stress Management
This program is available to all NSFHT patients who have concerns about their weight, blood pressure, blood sugar, cholesterol and would like to live a healthier lifestyle!
You can be referred to the program by your Physician, Nurse Practitioner or by calling the Family Health Team and registering.
