Our team consists of Certified Diabetes Educators specializing in diabetes prevention, care and management. The Diabetes Education Program is delivered and supported by an interdisciplinary team including: Registered Nurses, a Foot Care Nurse, Registered Dietitians, a Nurse Practitioner, and a Registered Social Worker.

This comprehensive program is available to all community members. Family Health Team patients are referred to the Diabetes Education Program by their Physician, Nurse Practitioner and by self referral.

The North Simcoe Family Health Team “Diabetes Education Program” provides patient-centred, ongoing support and education for adult patients focusing on self-management and SMART goals.

We provide individual, group and team appointments with the availability of flexible scheduling to meet your needs.

Visits to this clinic include education in areas such as:

  • Glucometer teaching and blood sugar review
  • Goal setting and developing action plans
  • Healthy eating
  • Foot care
