Diabetes Management Program
Who we are
Our team consists of Certified Diabetes Educators specializing in diabetes prevention, care and management. The Diabetes Program is delivered and supported by an interdisciplinary team including; Registered Nurse, Registered Dietitians, Nurse Practitioner, and Registered Nurse Mental Health Specialist.
This comprehensive program is available to all patients of the North Simcoe Family Health Team. Family Health Team patients are referred to the Diabetes Program by their Physician or Nurse Practitioner.
Program Overview
The North Simcoe Family Health Team “Diabetes Management Program” provides patient centered, ongoing support and education for the adult patient focusing on Self Management and SMART goals.
We provide individual group and team appointments with the availability of flexible scheduling to meet your needs.
What you can expect at your appointment with the Diabetes Management Program?
Visits to this clinic include education in areas such as:
- Glucometer teaching and blood sugar review
- Healthy eating
- Foot care
- Medication review
- Blood work ordering and review
- Blood pressure monitoring
- Depression screening
