Where is the North Simcoe Family Health Team located?
The North Simcoe Family Health Team is located at 240 Penetanguishene Road, Suite 100 in Midland.

Can I use your services without being a rostered patient of one of your physicians?
The services of the North Simcoe Family Health Team are only available to patients rostered to our physicians.

How is the North Simcoe Family Health Team run?
The Board of the NSFHT includes five physicians, elected by the member physicians. The Board has created an Advisory Committee of FHT Clinic and Program Leads.

What is an Electronic Medical Record?
Electronic Medical Record or EMR refers to a paperless, digital and computerized system of maintaining and managing patient health information. It is designed to increase efficiency and reduce errors. The
Electronic Medical Record system allows Family Physicians and other health care providers to chart patient health information using a computer. Functions such as ordering of medical tests, treatments, medications, and clinical guidelines used for the patient’s care are accessible to health care providers.
Electronic medical records systems allow health care providers to make safer, faster and more knowledgeable decisions about your health care.
Is my health information secure in an Electronic Medical Record?
Yes. Your Health Record is stored on a specialized computer system that uses "state-of-the-art" military grade encryption, such as what is used by banks and other financial institutions on the internet.
How do I find a Family Physician?
For patients who do not have a Family Physician, you can register with a service provider by the Ministry of Health & Long-Term Care called "Health Care Connect" by calling 1-800-445-1822 or visiting www.ontario.ca/healthcareconnect
How do I renew or replace my health card?
Locally, Midland Driver and Vehicle Licence Office at 20-9225 Hwy 93, Huronia Mall, Midland from Monday to Friday can provide the following services;
- Address changes on all cards
- Renewals of the green photo Health Cards
- Name changes on green photo Health Cards
- Replacement of lost or damaged photo Health Cards
- Voiding Red and White cares that are lost or damaged
- General inquiries – 705-526-1590
